What is Not Covered:

• Sterilization or reversal of sterilization

• Services related to diagnosis or treatment of mental disorders, mental illness or psychiatric condition

• Services related to diagnosis or treatment of chemical dependency

• Long-term rehabilitative therapy services

• Preventive care, including routine or periodic physical examination or immunizations

• Out-patient prescription drugs
