(Remember, most health care providers, doctors/hospitals, don't deal directly with the health insurance company.

They deal with a "Network Provider". The network provider is a "middleman" that has negotiated rates with the doctors and the hospitals.

Note well!   If you do not have health insurance coverage, the doctor will likely charge you $150 to $200 for the consultation / visit. However, if you do have health insurance, he has agreed with the network provider that he will only charge the contracted amount.  

Usually, this is currently in the range of $60 for the visit.   This is one of the compelling reasons to have health insurance coverage...you don't want to pay the retail price for a visit."
